We are seeking a skilled Advanced Practice Provider to join our multidisciplinary Field Team in delivering home- and community-based care to patients with complex needs.

The ideal candidate will have a strong background in geriatrics and/or palliative medicine, with experience caring for aging adults and adults with complex care needs.

Responsibilities will include delivering in-home clinical services, collaborating with the multidisciplinary team to cooperatively deliver transitions of care support, social determinants of health interventions, behavioral health evaluation and management, and complex medication management as clinically appropriate.

The successful candidate will also have the ability to build meaningful relationships with patients and their caregivers, delivering proactive and timely care aligned with what matters most to our patients in order to improve their quality of life and to avoid unnecessary ER visits and hospitalizations.

Key Responsibilities:

  • Deliver in-home clinical services to patients with complex needs
  • Collaborate with the multidisciplinary team to deliver transitions of care support, social determinants of health interventions, behavioral health evaluation and management, and complex medication management
  • Build meaningful relationships with patients and their caregivers
  • Deliver proactive and timely care aligned with patient needs
  • Participate in care team huddles, operational huddles, organizational meetings, educational training, and clinical partnership meetings

Requirements:

  • Graduation from an accredited health professions school
  • Successful completion of national board certification
  • Active DEA licensure and state-specific controlled substance registration, as applicable
  • Advanced training in geriatrics and/or palliative medicine preferred
  • Clinical experience caring for aging adults and adults with complex care needs required
  • Clinical experience caring for patients in the home setting including private homes and/or congregate living facilities preferred
  • Clinical experience providing virtual care via telehealth
  • Previous experience with managed care and/or value-based healthcare delivery preferred
