Job Description

We are seeking a skilled Mental Health Case Manager I to join our team. As a key member of our behavioral health department, you will provide case management services to patients and families to enhance their psychosocial functioning.

Responsibilities

  • Conduct thorough assessments to identify patient needs and develop effective treatment plans.">
  • Collaborate with interdisciplinary teams to coordinate care and ensure seamless transitions.">
  • Provide emotional support and education to patients and families to promote healthy coping mechanisms.">
  • Maintain accurate and detailed records of patient interactions and progress.">

Required Skills and Qualifications

To be successful in this role, you will need:

  • A Master's degree in Social Work, Counseling, or a related field.">
  • At least one year of experience in behavioral health.">
  • Excellent communication and interpersonal skills.">
  • Ability to work effectively in a fast-paced environment.">
  • Licensure as a Licensed Master's Social Worker (LMSW) or Provisional Licensed Professional Counselor (PLPC) is preferred.">
