Key Responsibilities- Perform comprehensive diagnostic assessments to identify the needs of our clients.
- Provide patient visits as scheduled, ensuring timely and effective treatment.
- Ensure treatment plans are up-to-date for each client, reflecting their evolving needs and progress.
- Complete comprehensive and succinct progress notes, documenting client interactions and outcomes.
- Communicate and follow up as needed regarding the client's status, collaborating with case managers and psychiatrists to ensure seamless care.
- Bachelor's degree in Social Work or a related field with one year of therapy experience or a Master's degree in Social Work or Professional Counseling.
- Social Work/Professional Counseling License or equivalent: LSW, LISW, LISW-S, LPC, LPCC, LPCC-S.
- Experience in therapy and counseling is preferred, with a focus on home-based settings.
- Valid driver's license, car insurance, and reliable transportation.
- Willingness to travel throughout assigned service areas.
- Comprehensive training to enhance your skills and knowledge.
