Empowering Patients to Maintain Independence

Our team of dedicated professionals includes nurses, physical therapists, occupational therapists, speech language pathologists, social workers, home health aides, and more who are committed to empowering patients to maintain their independence at home.

Key Responsibilities

  • Conduct initial assessments of patients and families to determine home care needs, including physical assessments and histories of current and previous illnesses
  • Supervise and direct Licensed Practical Nurses (LPNs) and Home Health Aides in implementing patient care plans
  • Develop patient education plans and direct LPNs in implementing them
  • Create and modify Home Health Aide care plans and supervise them through patient feedback and onsite interactions
  • Evaluate and revise patient care plans as patient status and needs change
  • Use health assessment data to determine nursing diagnoses and develop plans of care that establish goals based on nursing diagnoses
  • Administer medications and treatments as prescribed by physicians
  • Identify discharge planning needs as part of care plan development and implement prior to patient discharge
  • Act as Case Manager when assigned by Clinical Supervisor and coordinate patient care for assigned caseload
  • Prepare clinical notes as needed and communicate with physicians regarding patient needs

Requirements

  • Graduate of an accredited school of nursing
  • Active RN licensure
  • One to two years of recent acute care experience in an institutional setting
  • Bachelor's degree with one year of home health care experience preferred
  • Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order
