About the Role

Supplemental Health Care is currently seeking a skilled and dedicated travel nurse to join their team as a Home Health RN. This exciting opportunity will take you to Pittsfield, Massachusetts, where you will have the chance to make a meaningful impact on patients' lives.

This position offers a unique blend of independence and structure, allowing you to work autonomously while still being part of a supportive team. As a Home Health RN, you will be responsible for providing high-quality patient care in the comfort of their own homes, promoting health, and preventing illness.

Key Responsibilities
  • Provide comprehensive nursing care to patients with various medical conditions, focusing on prevention, education, and rehabilitation.
  • Develop and implement individualized care plans tailored to each patient's needs, goals, and preferences.
  • Maintain accurate and detailed records of patient progress, including treatment plans, medication administration, and any changes or concerns.
  • Collaborate with other healthcare professionals, such as doctors, therapists, and social workers, to ensure seamless care coordination.
  • Conduct regular assessments to identify areas for improvement and adjust care plans accordingly.
Requirements
  • OASIS/Medicare Certified Visits Experience
  • Electronic Charting experience
  • Minimum of 1 year of recent home health nursing experience (private duty does not qualify)
  • Current Massachusetts RN License
  • American Heart Association BLS
  • Valid U.S. Driver's License
Compensation and Benefits
