Job Summary

We are seeking a highly skilled Nurse Practitioner/Physician Assistant to join our team at Northwell Health. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and collaborating with our medical staff to ensure the best possible outcomes for our patients.

Key Responsibilities
  • Initial Patient Assessments: Conduct thorough initial patient histories and physical examinations to identify patient needs and develop effective treatment plans.
  • Plan of Care Development: Initiate, document, and communicate patient care plans, ensuring complete screening and preparation of patients for treatment.
  • Daily Patient Rounds: Perform daily patient rounds to monitor patient progress, address concerns, and make adjustments to treatment plans as needed.
  • Medication and Diagnostic Orders: Write orders for medications, laboratory work, and diagnostic tests, and interpret results to inform patient care decisions.
  • Collaboration and Communication: Confer with attending physicians, residents, nursing staff, and other care providers to ensure optimal quality of patient care.
  • Discharge Planning: Note discharge summaries, prescriptions, and referrals on patient charts, and inform patients of necessary post-discharge care instructions.
Requirements
  • Education and Certification: Master's Degree in a Nurse Practitioner Program and National Board Certification in specialty area required. Current license to practice as a Nurse Practitioner in New York State, and current license to practice as a Registered Professional Nurse in New York State, required.
  • Collaborative Practice Agreement: Has a collaborative practice agreement with designated protocols filed with NYS Department of Education OR collaborative relationship attestation as per the Nurse Practitioner Modernization Act 2015.
  • Certifications and Licensure: Current BLS certification, required. Current NCCPA Certification from the National Commission on the Certification of Physician Assistants (NCCPA), required.
