This role is responsible for delivering patient care through the nursing process, including data collection and assessment, identifying and prioritizing patient problems/needs, and evaluating the process and outcome of nursing care. The Pharmacist will coordinate, provide, and direct patient teaching activities and coordinate the care provided by health team members.
Responsibilities- Collaborate with the interdisciplinary care team to conduct initial and ongoing patient assessment, analyze assessment data, create a plan of care, implement treatment, and evaluate treatment effectiveness.
- Administer medications in a safe manner consistent with the State of Practice and Bon Secours Mercy Health policies and procedures.
- Serve as the point of contact for patients' care coordination throughout hospital departments.
- Act as a patient safety advocate by participating in ongoing quality improvement in the department.
- Process orders and provide medication review for appropriateness, safety, and efficacy during order processing and perform any necessary clinical support duties as needed.
- Ensure accurate preparation of compounded and non-compounded products, including safe handling consistent with regulations.
- Graduate of an ACPE accredited School of Pharmacy with a BS or PharmD.
- Advanced pharmacy degree - Doctor of Pharmacy, Masters, Residency (preferred).
- Maintains current license for pharmacist in the state(s) where practicing.
- Valid state-issued ID.
- BLS Basic Life Support (preferred).
- Immunization Certified (preferred).
- 2 years of recent experience in an acute care setting.
- 1 year of leadership experience (preferred).
- Experience in the field of pharmacy practice.
