Avante at Boca Raton is seeking a skilled MDS Coordinator to join our team.
Job Summary:The MDS Coordinator will be responsible for conducting and coordinating the development and completion of the resident assessment, Minimum Data Set (MDS), and Care Planning in accordance with current federal, state, and local standards, guidelines, and regulations.
Key Responsibilities:- Conduct and coordinate the development and completion of the resident assessment, MDS, and Care Planning.
- Ensure that all MDS Assessments are completed and transmitted to the State repository according to the regulations in the RAI Manual.
- Ensure that all residents meet skilled care coverage criteria per governmental or other regulatory requirements.
- Coordinate the review and revision of the resident's care plan by the interdisciplinary team after each quarterly review or other assessment.
- Monitor Quality Measures Report on a monthly basis with particular emphasis on those measures impacting the Federal Five Star System.
- Assist the nursing staff in encouraging the resident and his/her family to participate in the development and review of the resident's plan of care.
- Ensure that all personnel involved in providing care to the resident are aware of the resident's care plan.
- Participate in facility surveys (inspections) made by authorized government agencies.
- Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc.
- Current, unencumbered, active license to practice as a Registered or Licensed Practical Nurse in the state.
- Nursing Degree/Diploma from an accredited nursing school, college or university.
- Preferred, as a minimum, three (3) years of experience in a hospital, nursing care facility, or other related health care facility.
- Ability to read, write, speak, and understand the English language.
- Knowledge of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to nursing care facilities.
- Leadership and supervisory ability and the willingness to work harmoniously with and supervise other personnel.
