We are seeking a skilled Registered Nurse to join our team in Sherwood, providing high-quality care to patients in their homes. As a Registered Nurse in Home and Community Based Services, you will be responsible for delivering personalized care and support to patients, working closely with other healthcare professionals to achieve the best possible outcomes.
Key Responsibilities- Provide high-quality clinical services within the scope of practice and in accordance with infection control standards.
- Conduct thorough clinical nursing assessments to ensure patient eligibility and medical necessity for services.
- Develop and revise individualized plans of care and service plans in collaboration with other community providers.
- Ensure plan of care frequency and duration meets the needs of the patient and initiates plan of care revisions as needed with physician approval.
- A minimum of one year experience as an RN is preferred.
- Current RN licensure in the state of practice is required.
- Current CPR certification is also required.
