We are seeking a compassionate and skilled Licensed Vocational Nurse to join our team at NEXT CARE HOSPICE.

**Job Summary:**

As a Hospice LVN, you will provide high-quality patient care in a home or facility setting. Your expertise will ensure the appropriate use of medications, durable medical equipment, and supplies. You will educate and support patients, families, and caregivers, and participate in interdisciplinary team meetings to update the Plan of Care.

**Responsibilities:**

* Provide skilled nursing care in a home or facility setting
* Ensure the appropriate use of medications, durable medical equipment, and supplies
* Educate and support patients, families, and caregivers
* Participate in interdisciplinary team meetings to update the Plan of Care
* Provide compassionate and high-quality care to patients and families

**Qualifications:**

* Valid LVN license in California
* One year or more of experience in either Home Health or Hospice is preferred, but not required
* Previous experience in a hospice setting is a plus
* Ability to build rapport with patients
* Compassionate and caring demeanor
* Excellent written and verbal communication skills
