The position involves conducting comprehensive health assessments, reviewing medical history, medication adherence, social environment, and other risks. This provides unparalleled insight into a member's overall health and well-being, helping to improve quality of care and potentially close multiple care gaps with a single visit.

About the Position

This is a PRN/part-time position that offers flexible scheduling, allowing you to balance your personal and professional life. As a Clinical Nurse Practitioner, you will work collaboratively with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel to provide patient-centered care.

Responsibilities
  • Conduct adult/geriatric assessments, medical history, diagnosis, and treatment, as well as health education, physician referrals, and case management referrals.
  • Work with Primary Care Physicians (PCPs) on patient education and follow-up.
  • Maintain relationships with a nationwide network of advanced practice providers.
  • Services may be provided in various settings, including home visits, skilled facility visits, and telehealth visits.
Qualifications
  • A Master's degree or equivalent experience and satisfactory completion of NP licensure.
  • Current RN and NP licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority.
  • Board certification by the AANP, ANCC, or AACCN in a Matrix-approved specialty.
  • A valid BLS, ACLS, or CPR certification.
  • A minimum of 1 year of experience as a Nurse Practitioner.
  • Strong computer skills and familiarity with employee health/medical record software.
  • Excellent verbal and written communication skills with patients, clients, and colleagues.
  • Comfortable and flexible with frequent change.
Salary and Benefits

This role offers a competitive salary of $90.00 per completed assessment, with additional compensation for mileage reimbursement and no-show payments. You will also enjoy flexible scheduling, travel opportunities, and the satisfaction of making a positive impact in the lives of others.
