Job Summary

We are seeking a skilled and compassionate Registered Nurse to join our team as a Hospice RN/Case Manager. In this role, you will provide high-quality patient care, coordinate with interdisciplinary teams, and empower patients and their families to make informed decisions about their care.

Key Responsibilities:
  • Provide direct patient care services, including assessments, care planning, and implementation
  • Collaborate with physicians, nurses, and other healthcare professionals to ensure comprehensive care
  • Empower patients and their families through education and support
  • Manage patient records and maintain accurate documentation
  • Participate in quality improvement initiatives and contribute to a culture of excellence
Requirements:
  • RN license in the state of employment (or eligibility to obtain)
  • 1+ years of nursing experience in hospice, home health, or a related field
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Current driver's license and automobile insurance
