Requirements:
- Valid PCT Certification
- Current CPR/AED & CPI certifications
- Successful completion of a criminal background check and Medicaid exclusion list verification
- High School diploma or equivalent
- Support clients with daily living activities and assist with morning and evening care routines
- Monitor and report any changes in clients' physical, mental, or social conditions
- Ensure a clean, safe, and healthy environment for clients
- Maintain accurate and organized documentation of care provided
- Protect the confidentiality of clients and the organization
- Participate in mandatory training sessions as required by the organization
- Complete the necessary annual 12 hours of continuing education or training to retain certification
- Engage in physical tasks that may involve climbing, lifting, walking, balancing, stooping, and handling materials
- May assist in training new Patient Care Technicians
- Perform additional duties as assigned
- Basic literacy and numeracy skills
- Effective verbal and written communication skills
- Initial evaluation after 90 days; subsequent evaluations annually
