Job SummaryWe are seeking a skilled and compassionate Registered Nurse to join our team as a Home Health Care Nurse. As a member of our team, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.
Key Responsibilities- Assess and evaluate the physical, psychosocial, and environmental needs of patients and develop individualized care plans to meet their unique needs.
- Provide high-quality nursing care to patients in their homes, including administering medications, wound care, and other treatments as needed.
- Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care and coordination.
- Communicate effectively with patients, families, and caregivers to ensure that their needs are met and that they are informed and involved in their care.
- Document patient progress and outcomes in a timely and accurate manner, using electronic medical records and other tools as needed.
- Current, unrestricted state RN license.
- 1+ year experience in a clinical care setting as a nurse.
- Able and willing to travel within branch/office coverage area.
- Must have a dependable vehicle, valid driver's license, and current auto insurance in accordance with state laws.
- Associates Degree or Bachelor's Degree in Nursing is preferred.
- Previous Home Health or Hospice Experience is preferred.
