Job Summary

The Certified Nursing Assistant/Home Health Aide (CAN/HHA) role works under the direction of a Case Manager/DON and is responsible for providing resident care to assigned residents in accordance with resident assessments, care plans, and nursing management directives. This role collaborates with licensed care staff to coordinate resident care and adheres to Company nursing policies, procedures, and safety guidelines.
