Our team at St. Luke's Health System is seeking a skilled Registered Nurse to fill the role of Home Health Case Manager. This position requires strong communication skills and the ability to work effectively with patients, families, and healthcare teams to provide high-quality care in the comfort of the patient's home.

As a Home Health Case Manager, you will work closely with patients and their families to develop and implement personalized care plans that address their unique needs and goals. You will also collaborate with healthcare providers to ensure seamless transitions of care and provide ongoing support to patients throughout their recovery journey.

Key Responsibilities:

* Develop and implement personalized care plans for patients

Qualifications:

* Current RN license in the state of practice

* 2 years of RN experience, with hospice or acute care experience preferred

* Current Basic Life Support (BLS) certification

* Valid driver's license and proof of auto insurance
