About the Role

We are seeking a skilled Licensed Practical Nurse to join our team at Hackensack Meridian Health. As a key member of our multidisciplinary team, you will play a vital role in delivering patient-centered nursing and rehabilitation services to our residents.

Responsibilities
  • Administer medications and treatments as ordered by physicians
  • Assist in the supervision of nursing assistants and contribute to the development of plans of care for residents
  • Ensure accurate documentation meets state and federal guidelines, including charting, interventions, and medication administration
  • Perform and assist with direct patient care as needed, ensuring residents have proper equipment and adaptive devices
  • Coordinate resident care with other facility departments and assess changes in resident condition and needs
Requirements
  • Completion of an approved LPN program
  • NJ LPN License and current Continuing Education (CE) requirements
  • Ability to work effectively on a multidisciplinary team and serve as a patient advocate
  • Long-term care facility experience preferred, as well as experience with geriatric and disabled populations
  • Bilingual (Spanish) and CPR certification preferred
