We are seeking a skilled Licensed Practical Nurse to join our team at Hackensack Meridian Health. As a key member of our multidisciplinary team, you will play a vital role in delivering patient-centered nursing and rehabilitation services to our residents.
Responsibilities- Administer medications and treatments as ordered by physicians
- Assist in the supervision of nursing assistants and contribute to the development of plans of care for residents
- Ensure accurate documentation meets state and federal guidelines, including charting, interventions, and medication administration
- Perform and assist with direct patient care as needed, ensuring residents have proper equipment and adaptive devices
- Coordinate resident care with other facility departments and assess changes in resident condition and needs
- Completion of an approved LPN program
- NJ LPN License and current Continuing Education (CE) requirements
- Ability to work effectively on a multidisciplinary team and serve as a patient advocate
- Long-term care facility experience preferred, as well as experience with geriatric and disabled populations
- Bilingual (Spanish) and CPR certification preferred
