As a Licensed Physical Therapist (PT), you will play a crucial role in assisting patients to regain functionality, alleviate discomfort, and avert disabilities. Your responsibilities will encompass providing preventive care, rehabilitation, and treatment for individuals facing chronic conditions, recovering from surgery, or dealing with injuries, all while meticulously documenting patient progress.
KEY RESPONSIBILITIES:
1. Conduct comprehensive evaluations and assessments, promptly reporting findings to the physician and Clinical Manager, while formulating an effective plan of care for patients.
2. Implement physical therapy techniques, measurements, evaluations, and diagnostic tests within the home setting.
3. Maintain thorough documentation of initial evaluations, treatment plans, and re-evaluations of patient progress every thirty (30) days, adjusting the plan of care as necessary.
4. Complete detailed assessments and OASIS data in a timely manner, as required.
5. Provide orientation, supervision, and consultation to Physical Therapy Assistants.
6. Observe, document, and communicate with the Health Care Provider regarding the patient's treatment, responses, and progress, ensuring timely reporting of significant changes and obtaining necessary orders to modify the plan of care.
7. Collaborate with the Health Care Provider and other healthcare team members to ensure the delivery and coordination of high-quality physical therapy services.
QUALIFICATIONS:
- Master's degree in Physical Therapy.
- Active Physical Therapy license in the state of practice, in good standing.
- Minimum of one year of experience as a Physical Therapist.
- Valid driver's license, vehicle, and automobile insurance.
- Ability to conduct home visits and deliver physical therapy evaluations and treatments in a home environment, while effectively completing documentation and submitting it promptly.
- Experience with Home Health and Home Care Home Base documentation.
