Job SummaryWe are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team of dedicated healthcare professionals. As a LPN with Aveanna Healthcare, you will have the opportunity to provide high-quality, patient-centered care to individuals in the comfort of their own homes.
Responsibilities- Implement and manage patient care plans in collaboration with physicians and other healthcare professionals.
- Conduct thorough patient assessments and coordinate care to ensure optimal outcomes.
- Administer prescribed medications, treatments, and therapies as ordered by physicians.
- Document patient care and progress in a timely and accurate manner.
- Provide education and training to patients and their families on health promotion, disease prevention, and management.
- Collaborate with the care team to ensure seamless transitions of care and address any concerns or issues that may arise.
- Current, unencumbered LPN license in the state of practice.
- At least 6 months of hands-on nursing experience, preferably in home health care or a related setting.
- Current CPR certification from the American Red Cross or American Heart Association.
- Ability to provide care to patients of all ages, including infants, children, and adults.
