Job Overview

Tact Staff is seeking a skilled Social Worker to join their team for a travel assignment in Everett, Washington.

This is a unique opportunity to work in a Level II Trauma Center, providing complex discharge services to patients. The ideal candidate will have experience in guardianship, working with DHS, and brokering with outpatient services.

The successful candidate will be responsible for providing post-care planning, working with the formal discharge planner, and ensuring a smooth transition for patients. This role requires strong clinical skills, independence, and the ability to work well in a team environment.

Key Responsibilities:

  • Provide complex discharge services to patients
  • Work closely with the formal discharge planner to ensure a smooth transition
  • Develop and implement post-care plans for patients
  • Collaborate with outpatient services to ensure continuity of care
  • Provide education and support to patients and their families

Requirements:

  • MSW and BLS certification
  • LCSW preferred
  • 1 year of experience in social work
  • Ability to work independently and as part of a team
  • Strong clinical skills and problem-solving abilities
