Job Title: Physical Therapist

We are seeking a skilled Physical Therapist to join our team at Relient Health. As a Physical Therapist, you will be responsible for providing high-quality physical therapy services to patients in a hospital-based inpatient and outpatient setting.

Responsibilities:
  • Screen patients and develop individualized treatment plans
  • Perform evaluations and develop initial and interim treatment plans
  • Document progress toward identified objectives
  • Oversee Physical Therapy Assistant(s) and develop new programs/techniques to enhance physical therapy services
  • Conduct patient and family education
  • Deliver quality physical therapy services and prioritize patient care
Qualifications:
  • Graduate of an accredited Physical Therapy program
  • State licensure or eligibility
  • Stable work history and positive attitude
  • Skilled Nursing Facility (SNF) experience preferred
