Job Summary: We are seeking a skilled LPN Charge Nurse to join our team at North Hill Nursing and Rehabilitation Center, LLC. As a key member of our nursing staff, you will be responsible for providing high-quality patient care and leading a team of LPNs to ensure the best possible outcomes for our residents.
Responsibilities:
- Provide nursing care to residents in accordance with our facility's policies and procedures, ensuring their safety and well-being at all times.
- Make daily rounds to evaluate the physical and emotional condition of residents, identify their needs, and implement necessary interventions.
- Supervise and lead a team of LPNs, providing guidance and support to ensure the delivery of high-quality patient care.
- Develop and implement care plans that meet the unique needs of each resident, in collaboration with other members of the healthcare team.
- Communicate effectively with residents, families, and other healthcare professionals to ensure seamless care coordination.
Requirements:
- Graduate of an accredited School of Nursing and currently licensed as a Nurse in the State.
- Previous work experience in a long-term care setting, with supervisory experience and organizational skills preferred.
- Excellent technical assessment and documentation skills, as well as leadership qualities.
- Good communication skills, with the ability to communicate in English, both verbally and in writing.
