Unit Overview

Our facility spans 6 floors, providing care for both long-term residents and post-acute rehabilitation patients. Each floor accommodates 50-60 beds, with nurse-patient ratios varying by shift and patient population. We feature specialized units including dementia care, long-term care, and rehabilitation services. With a total of 250 beds, our designated units cater to specific patient needs such as general rehabilitation, neuro rehabilitation, traumatic brain injuries, and cancer care. Residents can engage in various therapeutic programs, including music therapy, expressive arts, and access to a garden room.

About the Rehabilitation & Nursing Center – Cedar

Designed with a homely atmosphere, our center offers a spacious environment compared to our previous facility. It includes modern amenities such as four expansive interior courtyards, a new dining area, an on-site dialysis center, an art room, and both private and semi-private rooms equipped with windows, restrooms, and showers. Our facility is furnished with contemporary decor and advanced technology to ensure comfort for our residents.

Position Responsibilities

Registered Nurse (RN)

The RN is responsible for delivering nursing care in a responsible and accountable manner, adhering to the standards set by the American Nurses Association (ANA) and the State of Michigan. This role involves utilizing the nursing process, which includes assessment, diagnosis, outcome identification, planning, implementation, and evaluation. The RN is expected to uphold professional performance standards, which encompass ethical practice, culturally competent care, effective communication, collaboration, leadership, education, evidence-based practice, quality improvement, resource management, and environmental health, all aimed at achieving optimal patient outcomes.

Licensed Practical Nurse (LPN)

Under the supervision of the RN or Mid-level Provider, the LPN collaborates with the healthcare team to contribute to patient care assessment, planning, implementation, and evaluation. The LPN provides information to patients and families regarding treatments, medications, and equipment used, ensuring coordination with the RN or Mid-level Provider. Documentation of care is maintained in accordance with established guidelines.

Typical Shift Patterns

  • 12-hour shifts: 6:30 a.m. to 7 p.m., with every third weekend
  • 8-hour shifts: 6:30 a.m. to 3 p.m., with every other weekend
  • Schedule accommodations may be available but are not guaranteed

Qualifications

Registered Nurse (RN)

  • Associate Degree in Nursing or completion of an LPN program required.
  • Bachelor's Degree in Nursing required within 5 years.
  • Current Michigan RN license required.
  • AHA or ARC Basic Life Support (BLS) certification required within 90 days.

Licensed Practical Nurse (LPN)
