Job Title: Occupational Therapist - SNF / LTC

Centra Healthcare Allied is seeking a skilled Occupational Therapist to join their team for a contract assignment in a SNF / LTC facility.

Job Summary:

The Occupational Therapist will be responsible for assessing patients' medical conditions, functional capabilities, limitations, and restrictions, and developing a treatment plan to improve their quality of life. The ideal candidate will have excellent communication skills, be able to work independently, and have a strong passion for providing high-quality patient care.

Responsibilities:
  • Assess patients' medical conditions and develop a treatment plan to improve their functional capabilities and quality of life.
  • Provide skilled occupational therapy services, including interventions and education, to patients and their families.
  • Collaborate with other healthcare professionals to ensure comprehensive patient care.
  • Document patient progress and maintain accurate records.
  • Participate in quality improvement initiatives to enhance patient outcomes.
Requirements:
  • Graduate degree in Occupational Therapy from an accredited program.
  • Current state license as an Occupational Therapist or in the process of obtaining licensure.
  • Current CPR certification.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
