About the Role

The Hyndman Area Health Center is seeking a skilled Licensed Clinical Social Worker to join our team of dedicated healthcare professionals. As a Licensed Clinical Social Worker, you will play a vital role in providing mental health services to patients experiencing mental health conditions, primarily in our MAT program.

Key Responsibilities
  • Provide mental health services to patients with mental health conditions, including assessment, diagnosis, and treatment planning.
  • Develop and implement effective treatment plans in collaboration with healthcare providers.
  • Document patient encounters and maintain comprehensive case notes.
  • Screen for and diagnose mental health and substance misuse conditions.
  • Utilize evidence-based interventions, assessments, and tools to support patient care.
  • Maintain current patient education materials and ensure compliance with administrative regulations and professional standards.
Requirements
  • Bachelor's degree in Psychology or related field; Master's degree preferred.
  • Continuing education in Substance Abuse treatment.
  • Minimum of five years of experience in social work, preferably in a healthcare setting.
  • Ability to use and understand computer applications.
  • Effective communication and interpersonal skills.
Licenses and Certifications
  • License to practice in the state of Pennsylvania.
  • CPR/AED Certification.
