Position Summary
- Administer medications to residents while adhering to Federal & State regulations and Senior Lifestyle protocols.
- Conduct admissions for new residents, including necessary assessments and documentation.
- Develop and implement both acute and long-term care plans tailored to individual resident needs.
- Assist in clinical system monitoring, such as tracking wounds or infection control, under the guidance of supervisory staff.
- Offer health-related consultations to residents and their families within the community.
- Communicate any changes in residents' health status to physicians and family members, ensuring accurate documentation.
- Respond promptly to emergency situations, demonstrating quick thinking and effective action.
- Maintain open lines of communication with all departments and team members to ensure coordinated care.
- Facilitate educational sessions for residents and staff to promote health awareness.
- Uphold the confidentiality of all resident information and documentation.
- Manage discharges, assessments, and clinical documentation as required.
Educational Background: Completion of an accredited practical or vocational nursing program
Field of Study: Nursing
Experience Required: Minimum of 1 year in senior care or a related field
Licensure: Current Licensed Practical Nurse's License in the practicing state; CPR Certification required
Language Proficiency: Sufficient English proficiency to perform job responsibilities and communicate effectively
Technical Skills: Basic computer literacy
Core Competencies:
- Ability to collaborate within a team and manage multiple tasks efficiently and safely.
