High Risk Clinician Position

We are seeking a highly skilled and experienced High Risk Clinician to join our team at Regal Medical Group. As a High Risk Clinician, you will be responsible for providing clinical care to our high-risk patient population in the San Fernando Valley and surrounding areas.
Job Summary:The High Risk Clinician will lead post-discharge transitional care efforts for select high-risk patient populations. This role will support high-risk post-discharge care for 100,000 high-risk members within the SFV Campus.
Key Responsibilities:- Provide clinical care to complex and high-risk patients in a designated clinic setting
- Lead interdisciplinary meetings to support patient care and develop care plans
- Adhere to evidence-based clinical guidelines and protocols
- Document all care provided in accordance with Regal Medical Group and regulatory compliance rules
- Refer patients to clinical, social, and behavioral health resources as needed
- Collaborate with primary care physicians and specialists to ensure seamless care coordination
- Doctor of Medicine or Doctor of Osteopathic Medicine degree
- Board Certified/Board Eligible in Internal Medicine, Geriatrics, or Emergency Medicine (preferred but not required)
- Active California license and DEA privilege
- Minimum of five years of prior clinical experience, with at least two years of managed-care or health-plan experience preferred
