Job Summary
We are seeking a highly motivated and compassionate Support Professional II to join our team. As a Support Professional II, you will play a vital role in advocating for the individuals we serve, promoting their independence, and supporting their overall quality of life.
Key Responsibilities
- Advocate for individuals with disabilities, ensuring their rights and needs are respected and met.
- Develop and implement person-centered plans, focusing on promoting independence, self-advocacy, and community integration.
- Provide support with activities of daily living, including personal care, meal preparation, and household management.
- Collaborate with interdisciplinary teams to ensure comprehensive care and support services.
- Communicate effectively with individuals, families, and colleagues to ensure seamless care coordination.
- Participate in ongoing training and professional development to enhance skills and knowledge.
Requirements
- High School diploma or equivalency preferred.
- Must be 18 years of age or older.
- One year of experience working with individuals with physical and/or developmental disabilities or a human services-related field, preferred.
- Excellent communication and interpersonal skills, verbal and written.
- Ability to coordinate activities and services.
- Ability to read, write, and speak the English Language in order to adequately interact and respond to the individuals and meet their service needs.
- Ability to be a Trained Medication Passer, within the first sixty (60) days of employment.
- Ability to be trained in CPR and First Aid within the first sixty (60) days of employment.
- Ability to see and hear adequately in order to make assessments and respond to individuals' needs.
- Ability to respond to emergencies according to established The Phoenix Residence policies and procedures.
- Ability to use a computer for required documentation, communication, and time log purposes.
