Excellent Post-Acute Care Medical Director Opportunity with Large Southeastern Healthcare Organization

Job Summary:
Hattiesburg Clinic, a leading physician-owned multi-specialty group in the Southeast, is seeking a Board Eligible/Board Certified Geriatrician, Internal Medicine or Family Medicine physician to assume a Medical Director role within our post-acute care program. The selected candidate will provide medical house services at community skilled nursing facilities and collaborate with advanced practice providers for in-home visits. Preference will be given to candidates with American Board of Post-Acute and Long-Term Care Medicine Medical Director Certification.
Key Responsibilities:
- Collaborate with the Quality Management team to develop and implement strategies to reduce skilled nursing facility length of stay and readmission rates.
- Provide medical leadership and guidance to advanced practice providers and registered nurses within the post-acute care program.
- Work closely with the hospital care service program and transitional team of RN case managers to ensure seamless care transitions.
Requirements:
- Board Eligible/Board Certified in Geriatrics, Internal Medicine, or Family Medicine.
- Experience in post-acute care and medical leadership preferred.
