We are seeking a highly skilled Public Health Specialist to join our team in Hendry/Glades County. As a Registered Nurse Specialist, you will play a vital role in providing home visitation services, health promotion, and referrals to at-risk pregnant women and infants.
About the RoleThis is an exciting opportunity to work with the Healthy Start program, implementing its standards and guidelines while collaborating with community agencies and service organizations. Your expertise in nursing and public health principles will be essential in developing Family Support Plans, providing education on child growth and development, and promoting basic health principles.
The successful candidate will have excellent communication skills, able to establish and maintain effective working relationships with clients, healthcare providers, and community partners. You will be responsible for outreach, initial contacts, assessments, and ongoing care coordination services for families via letter, phone calls, and home visits.
Responsibilities- Implement the Healthy Start program for families referred to and enrolled in Healthy Start in a manner consistent with the Healthy Start Standards and Guidelines and the Healthy Start Coalition contract.
- Provide services in a location that best meets the needs of the family through home visits, clinic visits, and visits in alternative settings.
- Develop Family Support Plans and provide education to families on child growth and development, nutrition principles, and the benefits of immunizations.
- Document service delivery as required and prioritize client caseload according to client risk status.
- Assist with Quality Assurance (QA) and Continuous Quality Improvement (CQI) programs and collaborate with community partners in the follow-up of referred at-risk families.
- Licensure as a Registered Nurse in accordance with Florida Statute 464 or eligible to practice nursing in accordance with Florida Administrative Code 64B9.
- Bachelor's degree OR three or more years of public health and/or maternal child health experience.
