The Nurse Care Manager coordinates all aspects of the patient's home care, interacting with other medical disciplines as needed and providing professional care to the patient. Key responsibilities include:
- Performing the admission visit to the patient in the home to determine eligibility for home care services and develop the plan of care to be followed.
- Coordinating all aspects of the patient's home care, including medical treatment and support services.
- Collaborating with other healthcare professionals to ensure comprehensive and high-quality care.
To be successful in this role, you will need:
- A graduate degree from an accredited diploma, Associate, or Baccalaureate School of Nursing.
- A current state license as a Registered Nurse.
- A current state Driver's License.
- At least one year of experience as a Registered Nurse, with two years preferred.
- Proof of current CPR and Hepatitis consent/declination.
- Reliable transportation with valid and current auto liability insurance.
