Job Overview

We are seeking a skilled Nurse Practitioner to join our team as a PRN/Part Time Clinician. In this role, you will conduct home risk assessments, medical history reviews, and provide diagnosis and treatment plans to patients. You will work collaboratively with physicians, case managers, and social workers to ensure comprehensive care.

Responsibilities

  • Conduct comprehensive health assessments, including medical history, diagnosis, and treatment plans.
  • Collaborate with healthcare teams to provide patient-centered care.
  • Develop and implement care plans to improve patient outcomes.
  • Provide education and support to patients and their families.

Requirements

  • Master's degree in Nursing or related field.
  • Current RN and NP licensure in state of practice.
  • Board certification in a relevant specialty.
  • Minimum 1 year experience as a Nurse Practitioner.
  • Strong communication and interpersonal skills.
