Key Responsibilities:
  • Develop and implement patient care plans in collaboration with our interdisciplinary team
  • Conduct ongoing assessments and evaluations to ensure patient needs are met
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination
  • Audit clinical records to ensure compliance with regulations and agency policies
  • Coordinate discharge planning and ensure a smooth transition for patients
