Key Responsibilities- Conduct comprehensive assessments of patients' needs and develop individualized treatment plans to improve their functional abilities.
- Collaborate with patients, families, and healthcare teams to ensure seamless care and effective communication.
- Provide skilled occupational therapy services, including therapeutic activities, exercises, and education, to patients in their homes.
- Evaluate patients' progress and adjust treatment plans as needed to ensure optimal outcomes.
- Communicate regularly with physicians, nurses, and other healthcare professionals to ensure comprehensive care.
- Supervise and educate Home Health Aides and Occupational Therapy Assistants as needed.
- Participate in agency activities, in-services, and meetings to stay current on best practices and industry developments.
- Master's degree in Occupational Therapy from an accredited program.
- Current Occupational Therapy License and BLS Certification.
- 1 year of experience as an Occupational Therapist, preferably in home health care.
