Healthmap Solutions - Lead Kidney Disease Management Specialist

About the Role

The Clinical Nurse Liaison will work with healthcare providers and patients to improve health outcomes for those engaged in Healthmap's Kidney Management Program.

Key Responsibilities
  • Liaise between Healthmap, provider practices, and patients to ensure positive engagement and performance.
  • Develop new relationships with healthcare providers and manage assigned caseloads.
  • Analyze data to identify opportunities to improve patient outcomes.
  • Educate healthcare providers on best practices, process improvements, and health management interventions.
  • Identify opportunities to educate patients on kidney health and related conditions.
  • Partner with physicians to identify patients who would benefit from care navigation support.
  • Advocate for patients and utilize community resources to support their needs.
  • Maintain thorough documentation of all interactions and meet organizational metrics and KPIs.
Requirements
  • Bachelor's degree in Nursing required.
  • Active, unrestricted RN license required.
  • 3+ years of experience in healthcare services, clinical operations, quality, or care management.
  • Experience in care gap closure or care coordination activities preferred.
  • Prior experience building relationships with healthcare providers or patients preferred.
