Role of the Ambulatory Clinical Pharmacist:
This role involves working Monday through Friday during daytime hours, with no weekend or holiday shifts required.

Remote Work Opportunity:
After a period of six months with the organization, there may be an option for this pharmacist to work remotely one day each week.

Key Responsibilities:
  • Implement SCL Health Pharmacy Services and SCL Health Medical Group initiatives.
  • Develop clinical pharmacy programs in accordance with established policies and regulations.
  • Review patient records to assess the appropriateness of medication therapies and identify candidates for outreach programs.
  • Evaluate patient conditions and address barriers to medication adherence to ensure comprehensive care.
  • Identify unaddressed health issues and refer patients to suitable healthcare providers.
  • Create effective medication management plans to minimize adverse effects.
  • Provide healthcare professionals with essential medication information to support safe prescribing practices.
  • Assess the outcomes of pharmaceutical interventions.
  • Collaborate with other healthcare professionals to optimize patient care.
  • Advise on the proper procurement, storage, and administration of medications in the clinical environment.
  • Prescribe and discontinue medications, adjust dosages, and order laboratory tests as permitted by collaborative practice agreements.
  • Maintain accurate documentation of medication plans and patient progress to ensure efficient care and support billing processes.
  • Engage in quality improvement initiatives by participating in data collection and analysis, leading to program enhancements.

Minimum Qualifications:
  • PharmD Degree.
  • At least two (2) years of experience in a retail pharmacy or ambulatory care setting, or completion of a pharmacy residency program.
Preferred Qualifications:
  • Completion of a PGY2 Ambulatory Pharmacy Residency or experience as an Ambulatory Clinical Pharmacist.
  • Board certification in a relevant area (e.g., BCACP, BCPS).
Physical Requirements:
Includes hearing, lifting, manual dexterity, pulling/pushing, vision, sitting, speaking, squatting/kneeling, standing, and walking.
