**Job Description**

As a Patient Care Coordinator at Montefiore Medical Center, you will play a critical role in ensuring that our patients receive high-quality care. Your responsibilities will include:

  • Demonstrating leadership skills in the coordination and delivery of patient care
  • Systematically assessing health care needs and responses to care rendered
  • Evaluating patient progress towards health goals and modifying critical path or plan of care accordingly
  • Developing, coordinating, and providing direct care for specific patient assignments
  • Providing a safe environment for patients and families

In addition to these key responsibilities, you will also be expected to communicate effectively with patients, families, and healthcare teams.

**Requirements**

To be considered for this role, you must have a BSN degree and 5 years of clinical experience, with two of those years spent in a specialty or related service. Certification in a specialty is preferred, as is a commitment to ongoing education and professional development.

**Salary**

$62,690 per annum, based on location and experience.
