Job Description

As a key member of the CommUnityCare team, the Advanced Practice Provider plays a pivotal role in delivering high-quality patient care. This individual is responsible for providing direct clinical services to patients, including comprehensive physical assessments, ordering diagnostic tests, and prescribing treatments.

The ideal candidate will possess excellent communication and interpersonal skills, with the ability to build strong relationships with patients, families, and staff. They will be proficient in using electronic medical records and have a keen eye for detail, ensuring accurate documentation and compliance with regulatory standards.

A minimum of one year of direct patient care experience as a provider is preferred, along with current certification in their area of specialty and unrestricted licensure to practice as a Physician Assistant or APRN in the State of Texas. The successful candidate will also hold a DEA license for prescribing purposes and Basic Life Support Certification through the American Heart Association.

Key responsibilities include:

  • Performing comprehensive physical assessments and developing treatment plans tailored to individual patient needs
  • Ordering and executing various tests, analyses, and diagnostic images to inform diagnoses and guide treatment decisions
  • Prescribing medications and therapies for preventive care and disease management
