Job Summary

Crawford Memorial Hospital seeks a skilled RN to join our Home Health team in a Weekend Days option position. The successful candidate will be responsible for assessing, planning, coordinating, implementing, and documenting care for an assigned caseload of patients.

Key Responsibilities:

  • Perform accurate and complete assessments of patients, including initial admission assessments and interdisciplinary screenings.
  • Develop and implement a home health plan that meets each patient's specific needs and is in compliance with regulating agencies, federal and state regulations, reimbursement guidelines, and agency policies and procedures.
  • Make comprehensive nursing decisions based on the interpretation of facts and evaluation of patient outcomes, and participate in care conferences to discuss multidisciplinary team responsibilities, patient progress, and plans for continued care.
  • Prepare and administer medications, start intravenous lines for medications and fluids, and maintain PICC/Central parenteral lines per protocol.
  • Assess the effectiveness of pain medications and revise the plan of care as indicated by the patient's response to treatment.
  • Ensure documentation is complete, accurate, and timely, and communicate documented information for evaluation.

Requirements:

  • Current Professional Licensure Required
  • Current IL RN License Required
  • BLS Required
  • Valid Driver's license and proof of personal auto insurance Required

Preferred Qualifications:

  • Acute Care Nursing minimum 1 year
  • Home Health Nursing minimum 1 year

Computer Skills:

  • Strong computer skills with the ability to navigate EMR programs

Additional Skills:

  • Excellent communication and customer service skills
  • Ability to work independently, prioritize, and complete tasks in a timely manner
  • Knowledge of state, federal, accreditation, and regulatory agency laws and requirements governing home healthcare
