Job DescriptionGreenLife Healthcare Staffing is seeking a Licensed Practical Nurse (LPN) to join our team in a Long Term Care setting in Lowville, New York. As an LPN, you will be responsible for providing high-quality patient care and working collaboratively with the healthcare team to ensure the best possible outcomes for our patients.
Responsibilities:
- Provide direct patient care, including administering medications, performing treatments, and documenting patient information
- Collaborate with the healthcare team to develop and implement individualized patient care plans
- Conduct patient assessments, identify potential health risks, and develop strategies to mitigate those risks
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination
- Participate in quality improvement initiatives to enhance patient care and outcomes

Qualifications:
- 2 years of experience in a hospital setting
- Valid license or certification in the state of practice
- Excellent communication skills
