Hospice RN Job Description

Gable Search Group is seeking a compassionate and skilled Hospice RN to join our team in Delaware County and Southern Montgomery County.

Key Responsibilities:

  • Conduct comprehensive assessments of patients and families to determine their needs and develop individualized care plans.
  • Admit patients for services and coordinate care with the interdisciplinary team.
  • Supervise Licensed Practical Nurses and Home Health Aids to ensure high-quality patient care.

Requirements:

  • Valid Registered Nurse (RN) license.
  • RN Diploma, Associates, or B.S. degree in nursing.
  • Minimum 1 year of Hospice agency experience.
  • Valid Driver's License, auto insurance, and reliable transportation.
  • Excellent observation, problem-solving, and communication skills.
  • Compassion and integrity.
