Job Title: RN/LPN Patient Care Advocate

HealthCare Support is seeking a skilled RN/LPN Patient Care Advocate to join our team. As a Hybrid RN/LPN Patient Care Advocate, you will play a crucial role in ensuring care gaps are closed, barriers to care are removed, and member and provider experiences are improved.

Responsibilities:
  • Conduct telephonic outreach to members to address gaps in care, barriers to care, and provide solutions to improve member experiences.
  • Pilot the Member Retention Program, addressing member concerns and identifying solutions to enhance the member experience.
  • Collaborate with the team to resolve member concerns and improve care outcomes.
  • Work with members to ensure care gaps are closed and barriers to care are removed.
Requirements:
  • Graduate from an accredited nursing school.
  • Must have RN/LPN license for PA.
  • Active and valid driver's license.
  • 2+ years of experience in patient outreach/health education within case management from another Managed Care Organization.
  • Motivational Interviewing skills.
  • Understanding of HEDIS measures and quality improvement initiatives.
  • MS Office, Smartsheet, Teams, and Zoom experience.
  • Good tenure in previous role.
  • WFH capability - smartphone, private workspace, high-speed internet.
  • Soft skills: strong communication skills, organizational skills.
