Job SummaryThe Private Duty Nurse LVN will provide one-on-one care to our patients in their homes, assisting with daily living activities and administering medications as prescribed. This is a rewarding opportunity to make a positive impact in the lives of our patients and their families.
Responsibilities- Implement physicians' prescribed plans of care
- Conduct patient assessments and coordinate care
- Administer prescribed medications, treatments, and therapies
- Document care provided in our system of record
- Teach and train family members on health promotion and disease management
- Communicate with physicians, clients, family members, and staff on patient problems, concerns, and changes in condition
- Active, unencumbered license (RN/LPN/LVN) in the state of practice
- Current CPR certification from American Red Cross, American Heart Association, or BLS
- TB skin test (current within last 12 months)
- Reliable transportation
