Position Overview

The Licensed Practical Nurse (LPN) is responsible for delivering high-quality care to residents while ensuring compliance with all relevant laws, regulations, and organizational standards. This role reports directly to the Director of Nursing or designated Nursing Supervisor.

Educational Requirements

High School diploma or General Educational Development (GED) equivalent.

Certificates and Licenses
  • Completion of an accredited LPN program is required.
  • Current LPN licensure in the state of Ohio is mandatory.
  • CPR certification must be obtained upon hire or during orientation and must remain current throughout employment.
Key Qualifications
  • Ability to work independently and effectively.
  • Knowledge of nursing practices, procedures, and applicable state laws governing nursing functions in long-term care settings.
  • Demonstrated leadership skills and a collaborative approach to working with LPNs, CNAs, and other staff.
  • Capability to implement the facility's programs, goals, and policies.
  • Understanding of and adherence to company policies, including those related to harassment and complaints.
  • Proficiency in reading, writing, speaking, and understanding the English language.
  • Ability to work as part of a healthcare team to assess resident needs, plan care, and implement interventions.
  • Skill in evaluating nursing interventions and collaborating with other healthcare team members as necessary.
  • Competence in administering prescribed medications and starting intravenous fluids, documenting accurately.
  • Ability to observe residents and report changes in their conditions, including adverse reactions to treatments.
  • Capability to provide basic resident care, including monitoring vital signs and assisting with personal hygiene.
  • Skill in sterilizing equipment and supplies using appropriate methods.
  • Ability to respond to residents' calls and determine appropriate assistance.
  • Competence in collecting samples for laboratory tests and preparing residents for examinations.
  • Ability to assist residents with mobility and daily living activities.
  • Skill in preparing food trays according to dietary requirements.
  • Ability to assist in emergency evacuations of residents.
