Job Summary

The RN Clinical Director is a key leadership position at LHC Group, responsible for overseeing and directing the patient care operations of our home care facility. This includes coordinating and completing assigned projects to support the company's objectives, overseeing patient eligibility, planning services, and supervising total home health care. The Clinical Director will also implement and maintain administrative practices, agency philosophy, goals, and policies to ensure compliance with state and federal regulations, enhance profitability, and provide motivation and retention of a qualified staff. Additionally, this role acts as a liaison with management staff and other departments throughout the company.

Key Responsibilities

• Supervise and ensure high-quality patient care services throughout the episode of care, including referral coordination, patient needs assessment, and development, implementation, and updates of the patient's individualized plan of care.
• Ensure coordination with physician, family, and other clinicians involved in care as appropriate, and communicate effectively with staff to ensure they are informed through staff conferences and mandatory in-services.
• Supervise patient care to ensure the delivery of safe, economical, and efficient patient care, adhering to relevant standards of accepted nursing and medical practice, federal and state regulations, and Joint Commission standards, as well as payer source criteria and LHC policies.
• Supervise agency staff workers' performance, monitor and assess employee performance regularly, and ensure that only qualified personnel are hired, with licensure verification, certification, and credentialing as required by law and policy.
• Assist with staff recruitment to ensure adequate staffing is maintained, operate within the staffing model, and serve as a member of the Management Team and QAPI Team according to required timeframes, implementing Performance Improvement Projects and annual strategic agency planning.
• Maintain and document an infection control program, participate in annual strategic agency planning, and ensure follow-up on any required action plans.
• Conduct at least monthly meetings with the medical director, approve medical director invoices, and submit them to the Home Office with appropriate supporting documentation for timely payment.
• Drive Operational metrics, including productivity, VBP, SVP, ACH, claims alert, late EMS, unverified services, orders audit, and agency financial statements to ensure the organization is on target with company goals related to operational efficiency, growth, quality, and finances.
• Act as Emergency Coordinator in the absence of the Executive Director/Alternate Executive Director during emergencies, ensuring appropriate plan execution, and communicate effectively and professionally with all customers to represent the agency and ensure timely and thorough investigation of concerns voiced by patients, caregivers, and referral sources.

Requirements

• If Clinical Director is an RN, must have current RN licensure in the state of practice, current CPR certification, and one year of supervisory and/or management experience in a home health setting or related healthcare setting.
• Current Driver's License and vehicle insurance, and access to a dependable vehicle, or public transportation.
