Home Health Physical Therapist Specialist

We are seeking an experienced Home Health Physical Therapist to join our team in Northfield, New Jersey. As a valued member of our healthcare team, you will be responsible for delivering high-quality physical therapy services to patients in their homes.
Job Summary- Evaluate and assess patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions.
- Develop and implement individualized treatment plans to improve and enhance patient well-being.
- Collaborate with healthcare teams to ensure comprehensive care coordination.
- Evaluate patient's functional status, body systems, and physical therapy needs, consulting with physicians to develop therapy plans.
- Provide services within scope of practice, following plan of care, and coordinating with healthcare teams.
- Monitor patient response to treatment and report changes to nurse supervisors and physicians.
- Treat patients to relieve pain, restore function, and maintain maximum functioning using evidence-based techniques.
- Instruct and inform participating team members, patients, and caregivers regarding plan of care, functional limitations, and progress toward goals.
- Visit patients across service areas, including nearby LHC agencies as needed.
- Complete progress notes and submit according to LHC policy.
- Supervise home health aides and PTAs per LHC policy and state regulation.
- Participate in staff conferences, committees, and education as requested by agency leadership.
- Schedule flexibility required to meet patient demands, including weekend rotations and observed holidays.
- Prepare discharge summaries and/or summary reports on patient condition every 60 days.
- Current Physical Therapy licensure in the state of employment.
- Current CPR certification.
- Valid driver's license and vehicle insurance, with access to a dependable vehicle.

Estimated SalaryThe estimated salary for this role is $80,000 - $110,000 per year, based on experience and location.
