Key Responsibilities:

The Speech Language Pathologist in Home Health is tasked with assessing and evaluating patient care needs related to speech and language disorders. This role includes:

  • Conducting comprehensive evaluations to determine the patient's functional abilities.
  • Developing and implementing individualized therapy plans in collaboration with the healthcare team.
  • Providing therapeutic interventions to enhance communication, swallowing, and cognitive skills.
  • Educating patients, families, and caregivers on effective communication strategies and therapy techniques.
