Key Responsibilities:
- Provide comprehensive medical assessments, order diagnostic tests, and formulate treatment plans for patients.
- Perform face-to-face visits in various settings, including home, skilled nursing facilities, and public locations.
- Collaborate with fellow nurse practitioners to develop best practices and improve patient care.
- Participate in community-based initiatives to address gaps in healthcare and build relationships with community members.
- Prescribe medications and perform procedures as necessary.
- Document patient information in electronic medical records.
Requirements:
- Master's degree in family health from an accredited nursing program.
- Active and unrestricted national certification from a recognized organization.
- Current state-issued license to practice as a Family Nurse Practitioner.
- Basic Life Support for Healthcare Professionals certification.
- Unrestricted driver's license.
Preferred Qualifications:
- 3-5 years of experience as a Registered Nurse and/or Nurse Practitioner, preferably in home health, community health, or public health settings.
- Experience with underserved populations and managing chronic conditions.
- Fluency in a language other than English is a plus.
