Job Title: Licensed Practical Nurse

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at IntelliChoice Home Care. As an LPN, you will provide one-on-one nursing care in the home to critical and complex clients, focusing on trach and/or vent patients of all ages.

Responsibilities:
  • Provide high-quality, patient-centered care in the home setting
  • Assess and develop individualized care plans to meet the unique needs of each patient
  • Collaborate with healthcare teams to ensure seamless care transitions and coordination
  • Administer medications and treatments as prescribed by physicians
  • Monitor patient progress and adjust care plans as needed
Requirements:
  • Current LPN license
  • CPR certification (no online classes accepted)
  • TB skin test (current within last 12 months)
