Home Health Physical Therapist PT

Pathwell Health is seeking a highly skilled Physical Therapist to join our team. As a Physical Therapist, you will play a critical role in helping our patients achieve optimal health and wellness.
Key Responsibilities- Assessment and Treatment Planning: Conduct thorough assessments of patients to identify their physical therapy needs and develop personalized treatment plans to address their goals and objectives.
- Rehabilitation and Conditioning: Implement and adjust rehabilitation and conditioning plans to help patients improve their mobility, strength, and overall physical function.
- Patient Education and Communication: Educate patients and their families on exercise programs and therapeutic procedures to be continued at home, and communicate effectively with healthcare teams to ensure seamless care.
- Documentation and Record-Keeping: Accurately document patient observations, interventions, and evaluations to ensure high-quality care and compliance with regulatory requirements.
- Collaboration and Teamwork: Work collaboratively with healthcare teams, including physicians, nurses, and other therapists, to provide comprehensive care and achieve optimal patient outcomes.
- Education and Licensure: Graduation from an approved Physical Therapy curriculum and current licensure as a Physical Therapist in the state of Virginia.
- Experience and Skills: Recent work experience in home health, strong clinical assessment and organizational skills, ability to handle multiple assignments simultaneously, and proficiency in using electronic devices for documentation and communication.
- Preferred Qualifications: Bilingual (Spanish) and experience working with diverse patient populations.
The Physical Therapist will work in a home health setting, providing care to patients in their homes. The ideal candidate will be able to work independently and as part of a team, and will be able to adapt to changing situations and priorities.
